This program allows you to request a routine screening mammogram at either Tawas St. Joseph Hospital or the Oscoda Health Park.
After submitting this request, a representative from our Central Scheduling Department will contact you by phone within 2 business days to confirm your appointment.
This program is available for routine screenings only. If you answer “Yes” to any of the following questions, we cannot schedule your appointment online. Please call our Central Scheduling Department at 989-362-9359 to schedule your mammogram.

√ Do you have breast implants or have you had a breast reduction?
This notice describes how medical information you submit through the online mammogram appointment request and pre registration feature of this web site may be used and disclosed. Also please review the St. Joseph Health System NOTICE OF PRIVACY PRACTICES for more information.
- All information submitted in the online appointment and pre-registration form will be treated by St. Joseph Health System as a confidential part of your medical record.
- The information you submit on this form is made available to St. Joseph Health System personnel for the purpose of scheduling your appointment and pre-registering you as a patient. By submitting this form, you are authorizing St. Joseph Health System to contact your insurance company for verification of coverage and payment.
PLEASE NOTE:
Upon arrival at either Tawas St. Joseph Hospital or the Oscoda Health Park, you will need to complete the admission process by:
- providing a copy of your physician’s signed order
- providing copies of your insurance card(s), and
- signing authorization forms
