What is Cardiac Rehab?
Cardiac Rehabilitation is an outpatient exercise/education program designed to provide support, physical training and education to the cardiac patient. The program is covered by most insurances including Medicare and requires a physician referral. Our core professional team includes a physician, a registered nurse, an exercise physiologist, and a dietitian. These professionals develop a treatment plan to help the cardiac patient improve his or her quality of life.
What Does the Program Offer?
Phase II is telemetry monitored exercise program. Your treatment plan will be designed on an individual basis and is dependent upon your medical diagnosis, initial fitness level, and your past medical history. The program provides personal, supervised and monitored telemetry, blood pressures and heart rates prior to exercise sessions as well as during the session and after cooling down. Exercise sessions are scheduled on Mondays, Tuesdays, and Thursdays.
Education is also provided. These classes discuss important issues as:
• Anatomy and Physiology of the Heart
• Cardiac Medications
• Stress Management
• Nutrition
• Smoking Cessation
• Risk Factors and Lifestyle changes
• Home Exercise
• Emotional Changes/Depression/Anxiety
What does the facility offer?
• Scott Care Tele Rehab monitoring system
• Bright atmosphere
• Terry cloth hand towels and water cooler are available
• Individualized training to improve strength and fitness
• Patients are encouraged to continue their exercise prescription through Phase III after completion of Phase II until they are comfortable to enter community exercise program, or they may choose to remain in Phase III, which is a self-pay maintenance program
Cardiac Rehab is designed for individuals who have experienced the following conditions:
• PTCA
• Congestive Heart Failure (Not covered by Medicare)
• Coronary Artery Bypass Surgery
• Heart Attack
• Heart Transplant
• Stable Angina
• Valvular Surgery
You will want to contact your insurance carrier to determine if Cardiac Rehab is a covered service for you. The insurance carrier may ask for a procedural code which is 93798 for Phase II Cardiac Rehab
What are the benefits of participating in a Cardiac Rehab Program?

What is Phase III Cardiac Rehab?
Phase III Cardiac Rehab is an exercise program monitored by Cardiac Rehab Staff. It is not covered by insurance. It is the next level after "graduating" from Phase II, or for those proactive individuals with risk factors for cardiovascular disease that want to begin a monitored exercise program. Monitoring consists of blood pressure and heart rate prior to exercise, during exercise and at cool down. Your exercise prescription is individualized and a physician referral is required.
For those individuals "graduating" from Phase II, the cost is $40.00 per month. The out of pocket cost to those entering into this level is $80.70 for the initial visit and $40 monthly. The initial visit includes a health history and physical assessment, individualized exercise prescription, and 1:1 training in regard to equipment orientation and safety, exercise goals and motivation, exercise intolerance warning signs, how to check your pulse and how to determine your target heart rate. Phase III exercise occurs on Mondays, Tuesdays and Thursdays from 9-11 am and 1-3 pm. A scheduled time is preferred.

Cardiac Rehabilitation Schedule:
Patient Orientation by Appointment, duration is approximately 2 hours
Exercise Sessions:
Scheduled on Monday, Tuesday, and Thursday
Sessions run for 1 hour at:
9 am, 10 am, 11 am, 1 pm, 2 pm, and 3 pm
Cardiac Rehabilitation is closed from 12 until 1 to permit staff lunch
